Records
Department Description
Responsible for ensuring that all medical records are maintained according
to state regulations and in compliance with JCAHO guidelines.
To obtain copies of Medical Records
You must fill out a Release of Information form and return it to us.
You may receive a form the following ways:
- Contact the Medical Records department at ext.253 and request that
a form be mailed to you;
- Download the form from the Forms
section of this site; or
- Come in to the Center and fill out the form in person.
Once the request is received, it generally takes 48-72
business hours to process your request. Your first copy is free, however,
each additional request for the same records results in a charge of
$1.00 per page.
We strive to process your request within 72 business hours, however,
extenuating circumstances may arise that could delay the processing
of your request.
HIPAA Compliancy
The Medical Records department upholds the utmost security of your medical
records. Protected health information is only provided when a proper
HIPAA-compliant release of information is obtained or for purposes of
treatment, payment or operations. Please review our Notice of Privacy
Practices for further information regarding the protection of your privacy
as a patient.
